Upon receipt of your completed form, E.A.R. will provide you with a Return Authorization Form # (RAF) along with instructions on where to send your return package. Please include this information along with the items you are sending to E.A.R. Please note E.A.R. is not responsible for lost or damaged packages due to the carrier selected.
